To the Former Participants in the Health Midwest Retirement Reserve Plan

When Health Midwest sold the majority of its assets to HCA in 2003, Health Midwest retained sponsorship of the Health Midwest Retirement Reserve Plan (the "Plan"). HCA did not assume any responsibility for this Plan.

Health Midwest (which changed its name to Community Health Group ("CHG") in 2003) terminated the Plan, effective May 31, 2004. To provide full funding for the continuation of all Plan benefits, CHG purchased an annuity contract from Transamerica Occidental Life Insurance Company ("Transamerica"). Effective June 1, 2004, Transamerica assumed the obligation to make all benefit payments under the Plan. If you are seeking information about a benefit you believe that you are owed under the Plan, please contact Transamerica Retirement Plan Services, using either the address, the toll-free telephone number, or the e-mail address below. You will want to refer to the Plan's contract number, which is 51519.

Re: Benefits Under the Health Midwest Retirement Savings Plan

When Health Midwest sold the majority of its assets to HCA in 2003, Health Midwest retained sponsorship of the Health Midwest Retirement Savings Plan - 403(b) and the Health Midwest Retirement Savings Plan - 401(k) (collectively, the "Plan"). HCA did not assume any responsibility for this Plan.

Health Midwest (which changed its name to Community Health Group ("CHG") in 2003) terminated the Plan, effective February 28, 2007. All participants were sent a mailing about the termination, and given an opportunity to elect where to move their accounts, or be paid in cash. All remaining benefits were distributed from the Plan during 2007.

If you have questions about the payment of your benefits from the 401(k) Plan #002 or 403(b) Plan #001, you may contact MetLife at (800) 856-7772.

If you did not respond to the Plan termination information we sent in 2007, and elect where to move your account,

  • If the account was with MetLife and FASCore, it was transferred to:
    • A MetLife IRA -- (866) 438-6477, option 2, option 2 - contract #4014776; or
    • A Group Annuity Contract with Transamerica Life Insurance Company - (800) 503-5978 - contract #92024.
  • If the account was with AIG, ING, Prudential, Mutual of America, contact:

Re: Social Security Administration
Potential Private Pension Benefit Information
If you receive a letter from the Social Security Administration telling you that you MAY be entitled to a pension benefit from Health Midwest, you received it in error. In 2008, we advised the IRS that all Plan benefits had been distributed when the Plans were terminated. In other words, the benefit described in the Social Security Administration letter has already been distributed or transferred as described in the two above memos.
